Electronic Claims Filing

For your convenience you are now able to file your C-3, Employer's Report of Industrial Injury, electronically. These forms can be signed electronically using Adobe Acrobat Approval or by contacting Associated Risk Management and providing a release form. To bring up your self-insured group's C-3, click on the appropriate group's link below.

(Note: In order for you to successfully complete and electronically email the forms below, you must have Adobe Acrobat Approval Version 5 or higher.)
